The use of imaging technology in the investigation of disorders of the head and neck has increased awareness of nodular thyroid disease in all areas of medicine. But are there enough important unanswered questions about thyroid cancer and has there been enough progress in the past decade to justify yet another textbook on the subject? The editors of Thyroid Cancer obviously believe so, and they have generally done an excellent job of putting together a concise book that highlights the advances and the controversies that surround them.

Tumors derived from thyroid epithelial cells are among the most common clinically recognized neoplasms. They include benign follicular adenomas, well-differentiated papillary or follicular carcinomas, poorly differentiated ``insular'' carcinomas, and dedifferentiated, or anaplastic, carcinomas. The spectrum ranges from benign occult papillary microcarcinomas to rapidly progressive anaplastic cancers. The changes in the epidemiology, classification, and worldwide pattern of detection of these thyroid neoplasms are sufficient to make this book a useful reference.

Unfortunately, the histology and molecular pathogenesis of nodular thyroid disease have been incorporated into one chapter, which does not provide enough detailed coverage of each subject. Our knowledge of the cause of thyroid cancer is just emerging, and there is increasing evidence that exposure to external radiation has a causative role; such exposure includes radiotherapy for cancers of the head and neck and treatment for acne and tonsillar disease. This is also observed in populations exposed to radioactive iodine fallout from nuclear disasters, most recently exemplified by the Chernobyl accident. Exposure to radioactivity there has had its highest impact in the young, and this is the subject of a separate chapter. Radiation has also been implicated in the RET-PTC gene rearrangement that is now considered to play a key part in the transformation of thyroid cells. The book also discusses the important role of dietary intake of iodine in the development of goiter and how iodine sufficiency may have contributed to the reduced incidence of follicular carcinomas.

The care of patients with tumors of thyroid follicular epithelium is well discussed. These tumors are generally amenable to surgical resection, but aggressive surgery is not usually indicated. The difficulty often lies in determining which patients should undergo surgery, when surgery should be performed in patients with uncertain diagnoses, and whether a complete thyroidectomy is needed, and the indications for prophylactic thyroidectomy in heritable diseases are detailed.

I highly recommend this book for trainees and practitioners in the fields of general medicine and surgery as well as specialists in endocrinology, oncology, and otolaryngology. Despite its compact size and relatively low price, it provides overviews of many highly relevant and common problems in the identification and care of patients with thyroid nodular disease.

In straightforward prose, Poundstone describes the roots of logic questions in interviews (the approach appears to have had its modern beginnings at Shockley Semiconductor Laboratory in 1957), drawing on the history of IQ testing in hiring interviews, psychological studies and interviews with Microsoft ex-interviewers and interviewees, makes a strong case for eliminating standard questions like "What are your strengths and weaknesses?" and replacing them with logic puzzles. Almost half of the book is devoted to an "answer" section, where Poundstone gives possible solutions to the brainteasers. In his debut novel, The Kite Runner, Khaled Hosseini accomplishes what very few contemporary novelists are able to do. He manages to provide an educational and eye-opening account of a country's political turmoil--in this case, Afghanistan--while also developing characters whose heartbreaking struggles and emotional triumphs resonate with readers long after the last page has been turned over. And he does this on his first try.

The Kite Runner follows the story of Amir, the privileged son of a wealthy businessman in Kabul, and Hassan, the son of Amir's father's servant. As children in the relatively stable Afghanistan of the early 1970s, the boys are inseparable. They spend idyllic days running kites and telling stories of mystical places and powerful warriors until an unspeakable event changes the nature of their relationship forever, and eventually cements their bond in ways neither boy could have ever predicted. Even after Amir and his father flee to America, Amir remains haunted by his cowardly actions and disloyalty. In part, it is these demons and the sometimes impossible quest for forgiveness that bring him back to his war-torn native land after it comes under Taliban rule. ("...I wondered if that was how forgiveness budded, not with the fanfare of epiphany, but with pain gathering its things, packing up, and slipping away unannounced in the middle of the night.")

Some of the plot's turns and twists may be somewhat implausible, but Hosseini has created characters that seem so real that one almost forgets that The Kite Runner is a novel and not a memoir. Besides the clear exposition of the Middle Pillar exercise, the book also contains an element borrowed from astrology - the seven principal ideas symbolized by the sun, moon and inner planets (every idea and thing can be classified within one of these root ideas). For example, the sun represents power & success, life, money, growth, etc., its positive color is orange, its negative color is yellow or gold, and its magical name is Jehovah Eloh ve Dah-as. So for example if one has a need of money, you would visualize the negative color (yellow/gold) as a next step following the Middle Pillar and vibrate the name. To help someone else who needs money, one would visualize the positive color (orange) and the person concerned and vibrate the name. I have seen the Middle Pillar described using six chakras (Ted Andrews)or all seven chakras where Regardie uses only five (he combines the Throat & Third Eye centres), but I suppose it doesn't matter, since the effect is the same. And the effect is immediate and very tangible. I found Marc Allen's afterword to be helpful (variations of and reflections on the meaning of the Middle Pillar), especially the description of the 2-minute version and the clear explanation of the tree of life. Unlike another reviewer (daniel j walton) I merrily mix eastern and western methods and have been chanting Sanskrit mantras after vibrating the Hebrew names in the middle pillar, and it enhances the feeling and the effect for me. I have to admit I was a bit disappointed that none of the three tales collected in Volume 7, the final set of "The Adventures of Tintin," constitute classic examples of Hergé's beloved comic book stories. But that seems a minor concern when you consider the epic scope of Hergé's body of work. It is not that these are bad stories, especially compared to the ones collected in Volume 1 of this series, but rather that Hergé so often provided classic tales, with Tintin traveling to the Moon or diving beneath the sea, that these final three adventures do not measure up.

"The Castafiore Emerald" begins with Tintin and Captain Haddock out for a walk and discovering a band of gypsies camped near the rubbish dump. This offends the good captain, who offers the gypsies the use of a large meadow near his hall. However, no good deed goes unpunished and he receives a telegram announcing the imminent arrival of Biana Castafiore, the Milanese Nightingale. Meanwhile, the broken step on the front staircase earns Haddock a badly sprained ankle and the opportunity to roll around the adventure in a wheelchair. The diva and her entourage then descend upon the hall, literally adding insult to injury by giving the captain the gift of a parrot. But as Castafiore repeatedly points out, she has brought along her jewels, including an emerald given the signora by the Maharajah of Gopal. The gypsy fortuneteller had already predicted the theft of the jewels and we expect her prophecy to come true, even though Castafiore is constantly yelling about her jewels missing. "The Castafiore Emerald" derives its comedy from the clash of characters with Tintin staying out of the way for the most part. Of course, by this time in the series Hergé is completely comfortable with his cast of characters, which shows in the interplay, Hergé also does a delightful take on that new fangled invention, the television.

"Flight 714" is sort of the generic Adventure of Tintin, with a little bit of everything that . A Qantas Boeing 707, Flight 714 from London touches down at Kemajoran Airport in Djakarta, java, last stop before Sydney, Australia. Disembarking is our hero, Snowy, Captain Haddock, and Professor Calculus. As they stretch their legs the good Captain spots a forlorn figure and slips a $5 bill into the man's hat. Once again no good deed of Haddock's goes unpunished and it turns out the old man is Mr. Carreidas, "The millionaire who never laughs." Well, Professor Calculus quickly takes care of that and Carreidas insists on flying Tintin and his friends to Australia on his special jet. Haddock is looking forward to a pleasure trip, an ordinary flight and no adventures, but fate has something else in mind, to wit: a hijacking, a cutting edge prototype means of transportation, an exotic island in the middle of nowhere, an evil scientist with truth serum, a gigantic stone head pagan idol, a threatening lava flow, the return of an old familiar villain, a space ship, and Tintin running around a lot with a gun. Pretty much all of these elements have popped up in the previous twenty Adventures of Tintin that Hergé had told over the previous decades. For that reason this particular adventure strikes me as more of a curtain call for Tintin and his friends than anything else, even though this is the penultimate tale and the Thom(p)sons are no place to be seen.

"Tintin and the Picaros" is the final adventure of Tintin, although there is not any sense of this being the end of the road (except for the surprising discover that suddenly Captain Haddock can no longer stand the taste of alcohol). As the story begins the Captain and Tintin are discussing the state of affairs in San Theodoros, when General Tapioca's dictatorship continues to rule in place of their old friend Alcazar. Then news comes that prima donna Bianca Castafiore has been arrested by Tapioca as part of a conspiracy to over throw the government. But when Tapioca charges Haddock, Tintin, and Professor Calculus as being part of the conspiracy a series of charges and countercharges, as well as outright insults, fly back in the forth in the headlines between Haddock and Tapioca. Finally the Captain agrees to accept Tapioca's "invitation" to come to San Theodoros to discuss the matter. Haddock is pretty much trapped into agreeing, and Calculus insists on going to Madame Castafiore's rescue, but Tintin refuses to go, knowing this has to be a trap. The title of the book refers to the Picaros, which is the name of the rebels in the mountains who want to take back the government of San Theodoros and return Alcazar to power. In this final Adventure of Tintin we are back on familiar ground for the most part, both in terms of the geography and the characters. We know, of course, that Tintin has not abandoned his friends and eagerly anticipate some clever way of arriving upon the scene at a most opportune moment. However, this turns out not to be the case, and when Tintin does arrive on the scene you know that Hergé is providing a standard adventure for his hero and his friends, and not something special.

But while "Tintin and the Picaros" and the other two tales found here are average adventure at best, there can be no doubt that taken together these 21 stories (23 if you count the two earlier "flawed" adventures) are a major accomplishment in the field of comic books. I only wish I had made a point of reading these classics two or three decades earlier, because with "The Adventures of Tintin" Hergé created one of the landmark comic book series since Cortes discovered pre-Columbian picture manuscripts in 1519. In terms of owning these stories your choice is between these smaller, hardbound books collecting three stories each, or the larger softcovered versions. I admit I first read most of them in the larger format but have the smaller hardback versions for the comic book section of my library.

In the Line of Fire is the most self-congratulatory, self-serving and arrogant books I've read in a long time, but what some people don't seem to grasp is that these reasons are what makes the book so entertaining to read. Of course the leader of a country that took power by military coup is going to produce a fictional auto-biography, but to complain about it so much misses the point. Know that you're getting a very specific version of the "truth" when you pick this book up, but at the sime time you're also getting a suspense filled page turner because you have no idea what he's going to say next.

Musharraf likens his own personal history to that of Pakistan's history as a nation, even going so far as to include commentary on the back cover that "the entire world depends upon him to succeed." While this a bit of a stretch to say the least, his story is entertaining. The complete random nature of much of the book can be frustrating for some, but it means never having to get bogged down in the details. The sections on his childhood are particularly entertaining, but the best part of the book by far is the story of how he came to power. According to Musharraf, he didn't come to power through a military coup. The Prime Minister in power in 1999 who was trying to get rid of Musharraf was the one responsible for the coup. Musharraf and the army carried out a "counter-coup" for the sake of the nation. Classic.

You may find yourself wondering why Musharraf wrote this book and who his intended audience was. This book was clearly meant for Americans. It wasn't written for his own country, and narcissism aside, for himself either. This book was written to convince Americans unsure of whether Musharraf is a "good guy" or "bad guy" that he is indeed on our side. He tries so hard to appear as America's friend in the book that it almost becomes painful at times. His desire to cultivate this image trumps the overal quality of the book because by the end, he's merely tacking on three or four page chapters on things like the emancipation of women or the "soft" image of Pakistan. The supreme irony here is that if enough of his opponents in Pakistan actually read this book, they might be able to muster enough support to overthrow Musharraf. This is the exact opposite image that he needs to present at home in order to stay in power.

Something else important to keep in mind is that Musharraf has the most unenviable job in the entire world. He has reactionary religious extremists at home that want to overthrow him on one hand, and the U.S. threatening to bomb Pakistan back to the stone age on the other. He's caught in a very tight spot with practically no room to maneuver. That he's been able to stay alive and in power for so long is truly remarkable. It's hard for me at least to be so critical of someone in that position. --By Lee L.

Pervez Musharraf has been the president of Pakistan since 1999. As a 4-year old, he moved to Karachi upon the partition of India and creation of Pakistan, and his lifespan and career has been tied to the life of his country ever since. After attending Pakistan's military academy he became a commando in the elite Special Services Brigade, fighting in the wars with India in 1965 and 1971. He rose through the ranks to become General and Chief of Army Staff in 1998. He became president in a dramatic confrontation with Prime Minister Nawaz Sharif, and has remained in office despite two assassination attempts.

As with other aspects of insurance licensing, specific instructions on how to register for your qualification exam are available from the insurance department. Ask for a licensing information bulletin or a licensing candidate handbook, which will describe where and when exams are given, the fees you must pay, and the testing procedures.

One thing all state insurance qualification exams have in common is that they are "closed book" exams. You will not be allowed to take any study materials or notes into the testing room. Even phones and calculators may not be allowed. In some states, the only items exam candidates are permitted to take into the testing room are their wallet and keys.

In most states, insurance qualification exams are given on computers. However, you will not need any computer or typing skills to take the exam. You will be instructed how to answer questions and given a short practice test to get comfortable with the equipment before the actual qualification exam begins.

When you complete a computer-administered exam, the software tells you immediately whether you've passed or failed. Your states will have rules for retesting in the event you don't pass. Those rules are described in your licensing information bulletin/candidate handbook.

I have hundreds of books and manuals on various techie subjects and Ellen's AutoCAD 2004 Bible ranks high on my list for books worth the purchase. I have used AutoCAD for years but after moving up to Acad 2005 from Acad 2000 I thought I could use some tutoring. The book didn't disappoint. For nearly every topic there is a step by step, hands-on, example that the reader can perform. There are around 160 drawing files on the accompanying CD; one for each example in the book. You can open that file in Acad and follow the steps in the book and experiment to your heart's content.

This is a really wonderful book. One of the reviewers is right that this book is available for free online. Just shows that price doesn't always correlate with quality. John H. Lienhard IV is internationally known for his work in heat transfer and thermal science, which spans some 40 years of research and teaching. He is a professor at the University of Houston and the creator of the program Engines of Our Intenguity which airs on public radio nationally. He is a member of the US National Academy of Engineering and has received dozens of awards for his research and teaching.
John H. Lienhard V is a professor at the Massachusetts Institute of Technology working in the fields of heat transfer and fluid mechanics. Computer viruses can be classified into several different types. The first and most common type is the virus which infects any application program. On IBM PC’s and clones running under PC-DOS or MS-DOS, most programs and data which do not belong to the operating system itself are stored as files. Each file has a file name eight characters long, and an extent which is three characters long. A typical file might be called “TRUE.TXT”, where “TRUE” is the name and “TXT” is the extent. The extent normally gives some information about the nature of a file—in this case “TRUE.TXT” might be a text file. Programs must always have an extent of “COM”, “EXE”, or “SYS”. Under DOS, only files with these extents can be executed by the central processing unit. If the user tries to execute any other type of file, DOS will generate an error and reject the attempt to execute the file. Since a virus’ goal is to get executed by the computer, it must attach itself to a COM, EXE or SYS file. If it attaches to any other file, it may corrupt some data, but it won’t normally get executed, and it won’t reproduce. Since each of these types of executable files has a different structure, a virus must be designed to attach itself to a particular type of file. A virus designed to attack COM files cannot attack EXE files, and vice versa, and neither can attack SYS files. Of course, one could design a virus that would attack two or even three kinds of files, but it would require a separate reproduction method for each file type. The next major type of virus seeks to attach itself to a specific file, rather than attacking any file of a given type. Thus, we might call it an application-specific virus. These viruses make use of a detailed knowledge of the files they attack to hide better than would be possible if they were able to infiltrate just any file. For example, they might hide in a data area inside the program rather than lengthening the file. However, in order to do that, the virus must know where the data area is located in the program, and that differs from program to program. This second type of virus usually concentrates on the files associated to DOS, like COMMAND.COM, since they are on virtually every PC in existence. Regardless of which file such a virus attacks, though, it must be very, very common, or the virus will never be able to find another copy of that file to reproduce in,and so it will not go anywhere. Only with a file like COMMAND.COM would it be possible to begin leaping from machine to machine and travel around the world. The final type of virus is known as a “boot sector virus.” This virus is a further refinement of the application-specific virus, which attacks a specific location on a computer’s disk drive, known as the boot sector. The boot sector is the first thing a computer loads into memory from disk and executes when it is turned on. By attacking this area of the disk, the virus can gain control of the computer immediately, every time it is turned on, before any other program can execute. In this way, the virus can execute before any other program or person can detect its existence.
